Why the wording is yours to supply
A permission or release form is a legal document, and what it must say depends on your state, your meeting place and your co-op's insurance. Peabody does not write that wording. This is a blank form, not legal advice. Have your co-op's own lawyer or insurer check the wording before families sign it.
Using the form
- Paste the wording your co-op agreed, or leave the box empty to print ruled lines and write it by hand.
- Print one copy a family. Each copy has lines for a child's first name or initials, an age band, the parent or guardian, a signature and the date.
- Keep the signed copies on paper, with whoever keeps the co-op's records.
What stays private
The co-op's name and wording are kept in this browser only. There is no box for a child or a family on this page: families write on the paper.
Running sign-ups too? The sign-up sheet, roster and volunteer hours print from the same kit, and the co-op fair tally scores a project night with codes, not names.
