What this page is, and is not

A blank page for the family's own planning. It is not an IEP, a services plan, or legal or medical advice. After a child leaves school to be homeschooled, the school's IEP usually stops applying; many families still like a written plan of their own. This is one, blank, to fill in by hand.

Filling it in

  1. Copy over the parts of school's last plan you want to keep working on.
  2. Write a few goals for the year, how you will practise each, and what it will look like when it is done.
  3. Tick what helps at home, and add your own.
  4. Look back every few months and write what changed.

Before or after withdrawing, read the IEP after leaving school: what federal law and your state say about services for a homeschooled child. Peabody Parents explains IEPs and 504 plans.

Nothing is typed on this page, so nothing about your child is kept anywhere.
