Things to do, with their deadlines
Give the local school board of your district a one-time notification (it may be a letter of intent). You do not send one every year.
Source: Utah Code 53G-6-204, school-age children exempt from school attendance (Utah Legislature)The board must acknowledge your notification.

Need the letter? Fill in the Utah letter of intent and print it.
All year
None are required. The statute says a local school board may not require a parent to maintain records of instruction or attendance.

Good habits (not in the law)
-
Keep a copy of everything you send, and of any acknowledgement you get back, with the date.
-
Start simple records on day one: the days you teach, the books you read and a few samples of work in each subject. They help if you move, return to school or apply to college.
